To comply with our clients’ vaccination policies and onboarding requirements, PDS is collecting vaccination status information from all employees, and in some cases, weekly COVID-19 testing results.

Please choose one of the below options, depending on whether you wish to report about your COVID-19 vaccination status or submit a COVID-19 test result.

When responding to this inquiry about your vaccination and/or testing status, please DO NOT submit any additional medical or family history information, including a reason for deciding to be vaccinated or not to be vaccinated. Please be advised that PDS may need to disclose your vaccination status and/or test results on a need-to-know basis to your assigned Client’s designated point of contact. In all other instances, all information received will be kept confidential in accordance with applicable law.
